sign off on drawings

Okay, for you guys. When you do a drawing, and have the client sign off that that is the sign to be built --- does anyone have a set statement tht is being signed --- something like,

"I hereby declare that this drawing has been approved for copy, layout, sizes, colors, etc, etc, etc on a particular date, etc, etc, etc??"

A line and a word underneath that line that says, APPROVED.

They sign it. That's it. Date it also.

Here is one I sometimes use, the language came from after compiling a bunch of other sign off/proof sheets through the years.
