Don't understand why he needed his crane truck to reach the top of the car. I would have used my step ladder.

Maybe the ladder they have isn't OSHA approved.

Did I read the related article right that one of the "braces" (I'm assuming outriggers) failed?

Good thing the car was there or the crane would have been completely flipped over.

I guess somebodys got some explaining to do.

That outrigger is neatly tucked in under the truck ... never was deployed.

I saw that but wondered if it collapsed back in when the truck went over. My guess would be like yours though, they didn't deploy it.
